Recognising the unique needs of those recovering from cancer treatment is fundamental to promoting quality of life moving forward. The effects of cancer treatments vary but can impact significantly on physical, emotional, psychological, financial, spiritual, professional and personal needs of an individual. There is a lack of awareness even among health professionals, that access to supports such as psychology, physiotherapy, exercise physiology, dietetics, specialised financial advice, employment assistance and education, is necessary for optimal recovery and gaining strength after treatment. No two patients are alike and a patient centred approach is the healthcare model we employ.
SEQ CCI is a patient and family centred care practice and our goal is to ensure every cancer patient in the region has a survivorship plan. We are passionate about helping patients and carers better understand treatment side effects, both short and long term, in order to identify the appropriate medical and allied health supports after treatment, when the regular contact with the treating team finishes.
It is important you know what supports are availble near your home. We endeavour to update our care providers regularly but the choice is always up to the patient. Allied health providers often book out well in advance so establishing a relationship early can help to ensure more timely appointments.
The GP is your principal healthcare provider and we liaise directly with him/her to co-ordinate your care. You decide who is a part of your survivorship team and we help facilitate optimal communication.
